Environmental Public Health Tracking (MA EPHT) Data Use Agreement

In preparing the data that exists in the MA EPHT network, every effort has been made to offer the most current, correct, complete, and clearly expressed information as possible. The data does not represent the original source data. It is strongly recommended that careful attention be paid to the contents of the metadata file associated with these data to evaluate data set limitations, restrictions, or intended use. The Massachusetts Department of Public Health shall not be held liable for improper or incorrect use of the data described and/or contained herein.

The information on the MA EPHT website is presented to describe the occurrence of health outcomes and select environmental data, but the information should not be used to establish causal relationships.

As a MA EPHTN user, I AGREE TO THE FOLLOWING:

• I have read and agree to the terms of use and limitations as described in the disclaimer page.
• The quality and completeness of the data cannot be guaranteed. I use these data at my own risk.
• I shall respect restrictions of access to sensitive data.
• I will report IMMEDIATELY any inadvertent or intentional identity disclosures or violations of this agreement of which I become aware to MDPH, BEH.
• I will comply with any additional terms and conditions of use set by the data provider. Where these exist they will be available through the associated metadata record.
• All data provided are the intellectual property of the organizations that provided the data and, therefore, the original source must be acknowledged.
• The presentation of the data does not imply the expression of any opinion by MDPH.

The MDPH, BEH is not liable for any damages resulting from the use of the MA EPHT network.
